Hospitalist and Post-Acute Group is a group of professional physician partners uniquely supported by exceptional solutions for the measurable success of our hospital partners. Our physicians are all Board Certified/Eligible in Internal Medicine with a primary focus on Inpatient Hospital Care.

We see our patients in the hospital and follow them through the course of their stay including the Intensive Care Unit, the medical/surgical units as well as the Acute Rehabilitation Center and Skilled Nursing Unit if necessary.

We work closely with case management to ensure the patient has an appropriate discharge plan and follow up is arranged. We communicate with the patient’s primary physician to ensure he or she has knowledge of the patient’s hospitalization. Teamwork is at the heart of our group. Exceptional patient care is the center of our focus.

Long-Term Acute Care

For patient who require complex medical management and therapeutic care 24 hours a day, 7 days a week. Upon transfer to our Hospitalist and Post-Acute Group, each patient is assigned an intra-disciplinary team that develops a personalized care plan under the direction of a physician.

Nursing and Rehabilitation Centers

Provide a full range of medical, nursing, rehabilitation and social services to treat and support each of our patients, whether long term or short term.

Rehabilitation and Therapy Services

Medical rehabilitation focuses on improving or restoring functional independence for individuals with disabilities resulting from injury, illness or a medical condition by providing physical and occupational therapy and speech-language pathology

Inpatient and Outpatient Rehabilitation

Where patients receive intense rehabilitation regimens to fully recover from an injury or illness and maximize their full potential

Home Health

As components of the post-acute continuum of care, our home health provide essential medical services including wound care and rehabilitation therapy. This specialized support allows patients to remain at home when they would otherwise have to be admitted to a costly institutional setting, such as a skilled-nursing facility or hospital.

Hospice

A family-oriented model of care designed to focuses on comfort and quality of life, rather than cure, for patients in the final phase of a terminal illness. We provide pain management and symptom control as well as emotional and spiritual support with the goal of helping each patient live every day as fully as possible.
