24-Hour Continuous Hospice Care

When someone you love is going through a tough time with a serious illness, keeping them comfortable at home is often the top priority. At times it may be medically necessary to elevate clinical care support if the patient experiences out of control pain or symptoms. That’s where continuous care comes in – a special kind of round-the-clock hospice service provided at bedside until pain and symptoms are managed. It’s an intensive, caring approach for those moments when a primary caregiver needs extra help managing acute symptoms. Every Medicare-funded hospice, including ours, is required to offer this level of care.

When Is Continuous Care Needed?

If your loved one is on hospice and facing a crisis, continuous care can step in to provide temporary 24-hour support at home for things like:

  • Severe pain that’s hard to control
  • Breathing difficulties
  • Persistent nausea, vomiting, or diarrhea
  • Changes in consciousness
  • Restlessness or agitation
  • Seizures

24/7 Care at Home brings extra hands-on support, including:

  • Round-the-clock clinical support from a hospice nurse or aide
  • Guidance from a hospice doctor who works with your loved one’s personal physician
  • Personalized, intensive care wherever your loved one calls home—whether that’s a private house, nursing home, or assisted living community
The Four Levels of Hospice Care

At 24/7 Care at Home, we offer four types of hospice care, as outlined by the Medicare hospice benefit, to meet your loved one’s needs:

  • Routine Home Care: This is the most common way we provide hospice care, with regular visits to homes, long-term care facilities, or nursing homes to keep patients comfortable and supported.
  • Continuous Home Care: This is our 24/7 intensive care, bringing skilled symptom management to your loved one’s bedside for up to 24 hours a day, following Medicare guidelines, until symptoms ease.
  • Inpatient Care: When symptoms can’t be managed at home, we provide round-the-clock care in a skilled nursing facility until your loved one is ready to return home.
  • Respite Care: For up to five days, your loved one can receive care in a skilled nursing facility to give their primary caregiver a much-needed break, without needing to meet inpatient symptom criteria.
