Home Health Care

Goal: Recover function, independence, and safety after an illness, surgery, or new diagnosis.

Hospice Care

Goal: Honor comfort, dignity, and time together when living with a life‑limiting illness.

What’s included

  • Physician‑led plan with skilled nursing for pain and symptom control
  • Home health aide for personal care and comfort
  • Medications, supplies, and equipment related to the hospice diagnosis (e.g., oxygen, hospital bed)
  • Medical social work, spiritual care, and volunteer companionship when available
  • 24/7 on‑call support for urgent needs
  • Respite care for caregiver relief and bereavement support for family

Who qualifies

  • A physician believes life expectancy may be six months or less if the illness follows its usual course
  • The focus is shifting from cure to comfort and quality of life

Coverage

  • Medicare covers hospice at 100% for eligible patients; Medicaid and many private plans are similar.
