Home Health Care in Costa Mesa, CA
Recovering at home isn’t only medical. When a Costa Mesa parent comes home from the hospital, families suddenly have practical worries too: how to get to appointments, whether Dad can still live alone, what programs exist and how to sign up. Medicare’s home health benefit includes help with exactly that kind of question, through medical social services. Many families don’t know it’s there.
24/7 Care at Home is a Vietnamese family-owned home health agency based in Westminster. We’ve served Orange County families since 2007, and our team visits Costa Mesa homes on the Westside, the Eastside and in Mesa Verde, as well as the neighborhoods near Fairview Park and South Coast Plaza.
What a medical social worker can help with
Medicare lists medical social services among the home health services it covers. When your provider includes them in your care plan, a social worker can help you:
- find community programs, such as meals, rides and caregiver support;
- think through whether home is still the right place long term, and what help that would take;
- deal with the stress and worry that come with a big health change;
- understand the paperwork that comes with a hospital stay and a new care plan.
Working alongside nurses and therapists
Social services work best as part of a team. A nurse may notice that a parent is skipping meals; a therapist may see that stairs are a problem. The social worker connects those observations with real resources, and the whole team keeps your doctor updated.
Hospitals Costa Mesa families come home from
Many Costa Mesa patients come home from Hoag Memorial Hospital Presbyterian in neighboring Newport Beach or from MemorialCare Orange Coast Medical Center in Fountain Valley. We mention them only as places families return from. We’re an independent agency.
The Costa Mesa Senior Center
The Costa Mesa Senior Center on West 19th Street is a strong local resource. According to the county’s 2026 senior center list, it offers support groups, health screenings, transportation, technology tutoring, a food distribution program, case management services and a lunch café. Our social worker can help you connect with it.

Costa Mesa questions
My mother lives alone near Fairview Park. Can home health still work? Often, yes. Home health doesn’t require someone to live with the patient, though it does help to have a family member or friend who checks in. Our social worker and nurse can look at what she needs between visits, such as a medical alert device, meal help or a neighbor who stops by, and help set it up.
Does the social worker visit every time? No. Visits follow your doctor’s orders, so a social worker may come once or twice while nurses or therapists visit more often. If a new worry comes up, tell any of us and we’ll pass it along.
