Home Health Care in Fountain Valley, CA
Fountain Valley is unusual: two full hospitals sit inside a fairly small city. UCI Health – Fountain Valley is on Euclid Street, and MemorialCare Orange Coast Medical Center is on Talbert Avenue. That means a lot of people here go home quickly after a knee or hip replacement, a fall, or a heart procedure, and then do the real work of recovery in their own living rooms. That’s where we come in.
24/7 Care at Home is a Vietnamese family-owned agency. Our office is just over the line in Westminster, and we’ve been helping Orange County families since 2007. Our therapists know the neighborhoods around Mile Square Regional Park and the Recreation Center and Sports Park well.
Physical therapy at home
A physical therapist works on what you need for daily life: getting up from a chair, walking safely to the bathroom, handling the two steps at the front door, building enough strength to get to follow-up appointments. Exercises are fitted to your real furniture and your real hallway, not a clinic gym.
Occupational therapy, and a rule worth knowing
Occupational therapy focuses on everyday tasks like dressing, bathing and cooking. Medicare has a specific rule here. Your doctor or provider must first order occupational therapy together with nursing, physical therapy, or speech-language pathology. After that, you can keep getting occupational therapy on its own, even once you no longer need the other services. (medicare.gov)
Who qualifies
Medicare covers home health when you need part-time or intermittent skilled care and you’re homebound. Homebound doesn’t mean bedbound. It means leaving home is hard because of an illness or injury, for example you need a walker, a cane, special transportation or another person’s help, and leaving takes a lot of effort. A doctor or nurse practitioner must see you in person and order the care.
Before you leave either Fountain Valley hospital, you can tell the case manager which agency you’d like. Medicare says your provider should give you a list of agencies serving your area.
Local help between visits
The Center at Founders Village, the city’s senior and community center on Bushard Street, is open to adults 55 and up. According to the City of Fountain Valley, it offers free HICAP Medicare counseling, a blood pressure check once a month, support groups, and resource referrals. Residents 60 and over can also apply for the city’s HOP ON senior mobility program, which offers curb-to-curb rides within Fountain Valley and up to a mile outside it. That can make outpatient follow-up much easier once home health ends.
