Home Health Care in Anaheim, CA
Anaheim is a big city with very different corners. A parent might live in a small house off Brookhurst Street in West Anaheim, in an older home near the Colony Historic District downtown, or up a winding street in Anaheim Hills. Wherever home is, the first days back from the hospital tend to feel the same: a new list of medicines, a body that tires quickly, and a family wondering who to call with questions. That’s the gap home health is meant to fill.
24/7 Care at Home is a Vietnamese family-owned agency with our office in Westminster. We have been caring for Orange County families since 2007, and our nurses and therapists visit homes across Anaheim.
Coming home from an Anaheim hospital
Anaheim has several hospitals, and families come home to us from all of them. Kaiser Permanente’s Anaheim Medical Center sits on East La Palma Avenue, close to the Anaheim Hills neighborhoods. West Anaheim Medical Center serves the west side, and AHMC Anaheim Regional Medical Center and Anaheim Global Medical Center are closer to the center of town. We mention them only so you know where our patients are usually coming from. We are not part of any of these hospitals.
Before discharge, tell the case manager or social worker which home health agency your family would like. Medicare says your provider should give you a list of agencies that serve your area, and the choice is yours.
What the first visit looks like
Medicare explains the order of things. Once your doctor or nurse practitioner refers you, the agency schedules a visit to talk with you about your needs and your health. Here is how that tends to go with us:
- A nurse or therapist comes to the house. They look at the medicines, the stairs, the bathroom and how your loved one is getting around.
- The care plan takes shape. Your doctor orders the services, and our team keeps your doctor updated on progress.
- Visits follow the doctor’s orders. That might be nursing for a surgical wound, physical therapy to rebuild strength, or teaching the family how to give an injection.
- Questions don’t wait for the next visit. A real person answers our phones 24 hours a day.
What Medicare covers, briefly
Medicare Part A and/or Part B can cover home health if you need part-time or intermittent skilled care and you are homebound. Covered services include skilled nursing, physical therapy, occupational therapy, speech-language pathology, medical social services, and home health aide help when you’re also getting skilled care. If you qualify, you pay nothing for the covered home health services themselves. Medical equipment is different: after the Part B deductible you usually pay 20% of the Medicare-approved amount. If your coverage comes through a Medicare Advantage plan, we’ll check your plan’s rules with you before the first visit.
Senior resources around Anaheim
Recovery is easier when a person isn’t isolated. The City of Anaheim runs the Downtown Anaheim Older Adult Center for people 55 and up, and the Brookhurst Community Center in West Anaheim has a lunch café for older adults. East Anaheim residents can use the East Community Center on Santa Ana Canyon Road. These are good places to come back to once your loved one is stronger.
