Home Health Care in Fullerton, CA

“But my dad still drives to church on Sundays. Does that mean he can’t get home health?” We hear versions of this question in Fullerton all the time. The word that trips people up is homebound. It sounds like someone who never leaves the house, but Medicare’s definition is more practical than that.

We’re 24/7 Care at Home, a Vietnamese family-owned home health agency with our office in Westminster. We’ve cared for Orange County families since 2007, and our nurses and therapists visit homes throughout Fullerton, from Sunny Hills to the older streets near Hillcrest Park and downtown, and the neighborhoods around Cal State Fullerton.

What “homebound” means to Medicare

Medicare says you’re homebound when both of these are true:

  1. Leaving home isn’t recommended because of your condition, or you have trouble leaving without help, such as a cane, walker, wheelchair or crutches, special transportation, or another person, because of an illness or injury.
  2. You’re normally unable to leave home, and leaving takes a lot of effort.

You can still leave for medical care. You can also leave for short, infrequent non-medical reasons, like attending religious services, and you can attend adult day care. So, a weekly ride to church with family doesn’t automatically rule anyone out. (medicare.gov)

The other conditions

Being homebound is one piece. You also need part-time or intermittent skilled services, such as nursing or therapy. A doctor or nurse practitioner has to see you in person, confirm the need, and order the care, and a Medicare-certified agency has to provide it. If you’d need more than part-time skilled care, home health isn’t the right fit, and we’ll say so.

Coming home from Providence St. Jude

Many Fullerton patients come home after a stay at Providence St. Jude Medical Center, the hospital on Valencia Mesa Drive. Others are discharged from AHMC Anaheim Regional Medical Center a few miles south. We name these only as places families come home from. We’re independent and not part of either hospital.

Ask the discharge planner for the list of home health agencies serving your area. If you’d like us, just tell them.

A local place to land

The Fullerton Community Center on West Commonwealth Avenue runs the city’s senior programs. The county’s 2026 senior center list notes recreational and health classes, a food distribution program, transportation, support groups and a lunch café. Once the visits taper off, it’s a good next step for staying active and connected.
