Community Liaison

JOB DESCRIPTION SUMMARY
The Community Liaison promotes 24/7 Care at Home’s mission by building strong relationships with healthcare partners, managing referral sources, and expanding territory presence. Key responsibilities include coordinating with internal teams to ensure smooth patient admissions, delivering high-quality service, and conducting educational outreach to community and facility partners. The role supports business growth, regulatory excellence, and consistent care delivery.

The duties and responsibilities of the Community Liaison will include, but not be limited to, the following:

  1. Promoting Mission and Building Relationships: Champion 24/7 Care at Home’s mission, vision, and policies in all interactions to ensure the highest quality of care. Cultivate strong relationships with physicians, hospitals, skilled nursing facilities (SNFs), and assisted living facilities (ALFs), educating them on home health and hospice regulations and fostering long-term partnerships.
  2. Referral Source Engagement: Identify and respond to referral source needs by providing regular patient care summaries to physicians and facility partners, enhancing care coordination and patient outcomes.
  3. Territory Management: Develop a robust strategy for managing accounts within your assigned territory, ensuring a consistent presence to maintain and expand business with existing accounts and discover new opportunities to meet established sales targets.
  4. Collaborative Service Excellence: Work closely with internal departments to deliver high-quality service to all customers—both internal and external. Facilitate seamless patient admissions by coordinating with clinical supervisors and nursing staff to align with patient care plans.
  5. Education and Community Outreach: Conduct educational sessions and in-services for community and facility partners, reinforcing 24/7 Care at Home’s commitment to regulatory excellence, quality care, and industry leadership.

POSITION QUALIFICATIONS

Minimum Experience and Education:

  1. Highschool or College Graduate
  2. At least two years of experience in healthcare sales and marketing. Good to have expertise in home health and hospice.
  3. Knowledge of home health, hospice, and palliative care practices and regulations.
  4. Bilingual skills in Vietnamese preferred

Compensation:

  1. Salary: 20 – 21/hour

Supplemental Pay:

  1. Bonus opportunities
  2. Commission pay
  3. Performance bonus

Work Schedule:

  1. Monday to Friday, with occasional weekends as needed.
  2. 80% on the field and 20% in the office
