Patient Care Coordinator
JOB DESCRIPTION SUMMARY
The Patient Care Coordinator is responsible for the day-to-day scheduling and coordinating of clinical staff shifts, taking client inquiries, performing non-medical in-home assessments, introducing care staff to clients, and resolving issues that may arise amongst the client, and their care team.
Essential Job Functions/Responsibilities:
The duties and responsibilities of the Patient Care Coordinator will include, but not be limited to, the following:
1. Orients and educates patients and their families by meeting them; explaining the role of the patient care coordinator; initiating the care plan; providing educational information in conjunction with direct care providers related to treatments, procedures, medications,
and continuing care requirements.
2. Develops interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression; fostering peer support; providing education to others regarding the case management process.
3. Monitors delivery of care by completing patient rounds; documenting care; identifying progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with patients in conjunction with the direct care providers; interacting with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults; reporting personnel and
performance issues to the unit manager; maintaining ongoing communication with utilization review staff regarding variances from the care plan or transfer/discharge plan.
4. Respects patients by recognizing their rights; maintaining confidentiality.
5. Maintains quality service by establishing and enforcing organization standards.
6. Maintains patient care database by entering new information as it becomes available; verifying findings and reports; backing up data.
7. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies.
8. Contributes to team effort by accomplishing related results as needed.
9. May assist in Intake, QA, Pre-billing, Administrator’s tasks.

POSITION QUALIFICATIONS
Minimum Experience and Education:
1. Graduate of an accredited vocational nursing program.
2. One to two (1-2) years nursing experience. Community health/home health or medical/surgical experience is preferred.
3. Currently, valid, non-restricted California Licensed Practical/Vocational Nurse license.
4. Possess and maintain valid CPR certification.
5. Complies with accepted professional standards and practice.
6. Understands philosophy of the home health concept.
Skills and Competencies:
• Excellent written and oral communication skills and public relations skills.
• Proficient in MS Productivity software (Word, Excel, Outlook, PowerPoint)
Continuing Education Requirements:
Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs.
