Home Health Care in the City of Orange, CA
The City of Orange has a lot of hospital beds for its size. UCI Health – Orange, the university medical center on City Drive, and Providence St. Joseph Hospital on West Stewart Drive both draw patients from all over the county, and Chapman Global Medical Center is on East Chapman Avenue. After a stay at a big medical center, a parent may come home with several specialists, new medicines and appointments on three different calendars. Part of our job is helping all of that make sense at home.
24/7 Care at Home is a Vietnamese family-owned home health agency based in Westminster. We’ve served Orange County since 2007, and our team visits homes across the city, from Old Towne and the Plaza to El Modena and Orange Park Acres.
One plan, many doctors
Medicare describes how this is supposed to work. Home health staff talk with your doctor or other provider about the care plan, and keep them updated on your progress. In practice, our nurses:
- compare the hospital discharge list with the medicines actually in the house;
- note what each specialist asked for and flag anything that seems to conflict;
- report changes, such as new swelling, a wound that looks different, or a fall, to the doctor who ordered the care;
- help you write down questions for the next appointment so they don’t get lost.
Who orders home health?
A doctor or other health care provider, such as a nurse practitioner, must see you in person and confirm you need home health care, then order it. Often that happens before you leave the hospital. Sometimes it happens a few days later at a follow-up visit, when it becomes clear that getting around the house is harder than expected. Either way, you can ask for the agency you prefer.
Senior life in Orange
The Orange Senior Center near Old Towne is a long-standing resource. The county’s 2026 senior center list notes technology tutoring, transportation and a lunch café among its services. It’s also a place where caregivers can ask about local programs.
Hospitals, mentioned plainly
We name UCI Health – Orange, Providence St. Joseph Hospital and Chapman Global Medical Center only because many of our patients come home from them. We are an independent agency and not affiliated with any hospital.
