Who qualifies for home health under Medicare

Medicare Part A and/or Part B covers home health when all of these are true (medicare.gov):

  1. You need part-time or intermittent skilled care. That means skilled nursing, physical therapy, speech-language pathology, or continuing occupational therapy.
  2. You are “homebound.” Medicare says this means both of these apply:
  3. leaving home isn’t recommended because of your condition, or you have trouble leaving without help (a cane, wheelchair, walker or crutches, special transportation, or another person) because of an illness or injury; and
  4. you’re normally unable to leave home, and leaving takes a lot of effort.
  5. A doctor or other provider (like a nurse practitioner) sees you, confirms you need home health, and orders it, and a Medicare-certified agency provides the care.

Still homebound if… Medicare allows trips for medical treatment and short, infrequent outings for non-medical reasons, like attending religious services. You can also still get home health if you go to adult day care.

Cost: you pay nothing for covered home health services. After the Part B deductible, you pay 20% of the Medicare-approved amount for covered medical equipment. If you have a Medicare Advantage plan, the plan’s own home health rules apply, so check with the plan. We can help you check.

What Medicare home health does not cover: 24-hour care at home, meal delivery, homemaker services (shopping, cleaning) unrelated to your care plan, and personal care alone (help with bathing or dressing) when that’s the only care you need.

CMS lists 24/7 Care at Home as offering all six Medicare home health service types (Provider Data Catalog, Home Health Care Agencies, October 2026 release):

Speech-language pathology Speech, swallowing and thinking skills, for example after a stroke
Medical social services Help with the emotional, financial and community-resource side of an illness

How to start (no referral needed to call)

  1. Send a request. A family member can call directly. You don’t need a referral to call us, and a real person answers 24/7. Online requests get a call back within minutes. [btn]
  2. We handle the doctor’s order. If home health makes sense, we contact your doctor (or the hospital or skilled nursing discharge planner) for the order Medicare requires. If you’re still in the hospital, tell the social worker or discharge planner that you want 24/7 Care at Home. Medicare says the provider should give you a list of agencies that serve your area, and you can choose.
  3. Care can start within 24 hours. A nurse or therapist comes to the home for the first visit, reviews medicines and safety, and sets up the plan of care with your doctor.

Vietnamese-speaking nurses (Y tá nói tiếng Việt)

We are a Vietnamese family-owned agency, and families can ask for a Vietnamese-speaking nurse. That matters most at the hard moments: going over a new medicine list, explaining a wound-care routine to the family member who will help each day, or helping a parent tell the nurse what hurts in the language they think in.

Frequently asked questions

Do we need a referral before we call? No. Any family member can call us directly, and a real person answers 24/7. Medicare does need a doctor’s (or other provider’s) order before visits begin, and we contact the doctor for you.

How fast can home health start? Care can start within 24 hours. If you send a request online, we call you back within minutes.

Does Medicare pay for home health? If you qualify (you need part-time or intermittent skilled care, you’re homebound, and a provider orders it), you pay nothing for covered home health services. Covered medical equipment is 20% after the Part B deductible. If you have a Medicare Advantage plan, check that plan’s rules. We can help you check.

My mother still goes to temple, church or adult day care. Can she get home health? Possibly. Medicare says short, infrequent absences for things like religious services don’t rule you out, and you can still get home health if you attend adult day care.

Is home health the same as a caregiver who stays all day? No. Home health is part-time skilled care: visits from nurses, therapists and aides. Medicare doesn’t pay for 24-hour care at home, or for personal care alone when that’s the only care needed.

Can we choose our agency when Dad leaves the hospital? Yes. Medicare says the provider should give you a list of agencies that serve your area. Tell the discharge planner you’d like 24/7 Care at Home.

Can we ask for a nurse who speaks Vietnamese? Yes. Ask when you call.

What do your CMS star ratings mean? CMS gives home health agencies a “Quality of patient care star rating” from 1 to 5, based on measures like timely start of care and improvement in walking, bathing and breathing. In the October 2026 CMS release, our rating is 5. Our “HHCAHPS Survey Summary Star Rating”, which comes from patient surveys from April 1, 2025 to March 31, 2026, is 4. You can check both on Medicare Care Compare.

Which cities do you serve? Westminster, Garden Grove, Santa Ana, Huntington Beach, Fountain Valley, Anaheim, Irvine and Costa Mesa, among others. Send a request to check your address.
