Home Health Care in Los Alamitos, CA
Los Alamitos is a small city with a full hospital in the middle of it. UCI Health – Los Alamitos sits on Katella Avenue, a few minutes from most homes in town and in neighboring Rossmoor. When a discharge happens there, it can happen fast, sometimes with only a few hours’ notice. A short list of questions can make that day much calmer.
24/7 Care at Home is a Vietnamese family-owned home health agency. We work from our office in Westminster and have helped Orange County families since 2007, including households on both sides of the Joint Forces Training Base and up toward Cypress.
Questions to ask before you leave the hospital
Bring this list to the case manager or discharge planner:
- Does my doctor think I need home health? A doctor or nurse practitioner has to confirm it after seeing you in person and order the care.
- Which services are being ordered? Nursing, physical therapy, occupational therapy, speech therapy, or a mix.
- May I see the list of agencies that serve my area? Medicare says your provider should give you one.
- Does the hospital have a financial interest in any agency on the list? Medicare requires your provider to tell you.
- When should the first home visit happen, and who will call to schedule it?
- Which medicines changed? Ask for a printed list to keep on the fridge.
Your choice of agency
You don’t have to take the first name on a list. If you’d like us, tell the case manager “24/7 Care at Home” and we’ll coordinate the referral. After that, our team calls to set up a first visit, talks with you about your needs, and keeps your doctor updated on the care plan.
After you’re home
Many Los Alamitos patients also come home from MemorialCare Long Beach Medical Center or La Palma Intercommunity Hospital. We name local hospitals only as places families return from; we aren’t affiliated with any of them.
The Los Alamitos Community Center on Oak Street hosts the city’s senior programs. The county’s 2026 list mentions recreational classes, information and referral, a food distribution program, support groups and a lunch café. It’s a short drive from almost anywhere in town.
