Home Health Care in La Palma, CA

La Palma is one of the smallest cities in Orange County, a quiet grid of homes around Central Park with La Palma Intercommunity Hospital right on Walker Street. Being small has an upside: almost everything is close. But when a parent comes home from the hospital, the questions are the same as anywhere else. How often will the nurse come? How long will they stay? Who covers the rest of the day?

24/7 Care at Home is a Vietnamese family-owned home health agency based in Westminster. We’ve been caring for families across Orange County since 2007, and La Palma is a quick drive for our team.

How often do visits happen?

Medicare’s answer has a few parts, and it helps to see them together:

  • If you qualify, there’s no set limit on the number of home health visits. Care continues as long as your provider keeps ordering it and you keep meeting the conditions.
  • “Part-time or intermittent” usually means skilled nursing and home health aide services combined for up to 8 hours a day, with a maximum of 28 hours a week.
  • For a short time, if your provider decides it’s needed, you may be able to get more frequent care: under 8 hours a day and up to 35 hours a week.
  • Visits follow the doctor’s orders. Our staff should visit as often as your provider orders. (medicare.gov)

In real life, most people get a few visits a week from a nurse or therapist, each lasting under an hour or so, rather than long shifts.

Planning the rest of the day

Because home health isn’t round-the-clock care, it helps to sketch a simple weekly plan with your family:

  • Who is home in the mornings when medicines are due?
  • Who can drive to follow-up appointments?
  • Who can stay overnight for the first few nights, if needed?

Our nurse will help you fill in that plan at the first visit, and we’ll adjust visit times where we can so a family member can be there.

Close-by help

The La Palma Community Center on Walker Street runs the city’s older adult programs. The county’s 2026 senior center list notes recreational classes and information and referral services. It’s a good first call when you need to find meal programs or transportation in the area.

Hospitals La Palma families come home from

Besides La Palma Intercommunity Hospital, many La Palma residents are discharged from UCI Health – Los Alamitos or from Providence St. Jude Medical Center in Fullerton. We mention these only as places people come home from. We’re an independent agency.
