Home Health Care in Yorba Linda, CA
Yorba Linda families tend to be close-knit, and many parents here have a full weekly rhythm: church or temple, a standing lunch with friends, a class at the community center. After a hospital stay, one of the first worries is whether getting home health means giving all that up. Usually it doesn’t, and Medicare spells out why.
24/7 Care at Home is a Vietnamese family-owned home health agency. We’ve served Orange County since 2007 from our office in Westminster, and our nurses and therapists visit homes across Yorba Linda, from the streets near the Richard Nixon Presidential Library and Museum to the neighborhoods around Savi Ranch at the east end of town.
What you can still do while getting home health
To qualify for home health, Medicare says you must be homebound: leaving home takes a lot of effort, and you normally can’t leave without help. But Medicare also says:
- you may leave home for medical treatment;
- you may leave for short, infrequent non-medical reasons, like attending religious services;
- you can still get home health if you attend adult day care.
So a Sunday service with family, or a regular day program, doesn’t have to stop. Just let our nurse know about the routine, so visits can be scheduled around it.
For caregivers who work
Many adult children in Yorba Linda are juggling a job, kids, and now a parent’s recovery. A few things that help:
- Pick a “point person” in the family who gets the updates and shares them.
- Ask for visit windows that line up with when someone can be home. We’ll do our best.
- Use a simple notebook by the bed for nurses, therapists and family to leave notes.
- Call us any hour. A real person answers, which matters at 10 p.m. when a question can’t wait until morning.
Yorba Linda resources
The Yorba Linda Community Center on Casa Loma Avenue runs the city’s older adult programs. The county’s 2026 senior center list notes transportation, support and a lunch café among its services. It’s also a good place to look for caregiver support once home health is done.
Hospitals our Yorba Linda patients come home from
Kaiser Permanente’s Anaheim Medical Center on East La Palma Avenue and Providence St. Jude Medical Center in Fullerton are the hospitals many Yorba Linda residents use. We mention them only as places families come home from. We’re an independent agency.
